If you wish to register with Health Te Aroha for your medical services, please download our enrolment forms:

• Patient Enrolment Form
• Request for transfer of medical notes

Once you have printed and completed the forms, please bring along to our clinic with identification, where your registration will be processed.

You are also welcome to collect an enrolment form directly from reception.
